Heart Murmur in Puppies: Understanding Causes, Symptoms, and Treatments

Heart murmurs in puppies are a common finding during veterinary check-ups. Whether benign or serious, understanding the potential causes, symptoms, and available treatments is essential to ensuring your furry companion grows up with a strong and healthy heart

What is a Heart Murmur in Puppies?

A heart murmur is an abnormal sound heard during the heartbeat, caused by turbulence in the blood flow within the heart chambers or vessels. Normally, blood flows in a smooth, well-defined manner. However, certain conditions can disrupt this flow, creating turbulence that is audible with a stethoscope.

Why Do Heart Murmurs Occur in Puppies?

Several factors may explain the presence of a murmur:

  • Changes in Blood Composition: Anemia (thinner blood) or severe dehydration (thicker blood) can cause turbulence.
  • Valvular Abnormalities: A heart valve that doesn’t close properly allows blood to flow backward, producing a murmur.
  • Congenital Heart Defects: Structural heart abnormalities interfere with normal blood circulation.
  • Altered Heart Muscle Contraction: Abnormal contraction of the heart muscle can also be responsible.

Benign or Pathological Murmur?

A heart murmur can be:

  • Physiological (Benign): Not linked to any abnormalities and common in growing puppies.
  • Pathological: Resulting from functional or structural heart anomalies.

Evaluating the Murmur: The Question of Intensity

Heart murmurs are graded from I to VI. Each grade represents the intensity of the murmur. Grade I is associated with a very faint murmur, heard in only one specific location. The higher the grade, the more easily the murmur can be heard in all areas where the heart is audible.

In cases of a grade VI murmur, it can even be heard with the stethoscope lifted off the chest wall. However, it is very important to note that the intensity of the murmur is not correlated with the severity of the lesion. For instance, a dog with a very serious heart condition may have a grade I/VI murmur, while a dog with a grade VI/VI murmur could have a minor lesion.

Important: The intensity of the murmur is not necessarily indicative of the severity of the heart condition. A faint murmur can hide a severe pathology, while a loud murmur may result from a minor anomaly.

Why is it Crucial to Diagnose a Murmur in Puppies?

In puppies under six months, a murmur may be the first sign of a congenital heart defect. Early detection allows for:

  • Correcting certain anomalies and ensuring a normal life.
  • Preventing progression to heart failure or premature death.
  • Preparing the owner for incurable conditions, avoiding a sudden revelation during a heart crisis.

Distinguishing Physiological from Pathological Murmurs

In some cases, auscultation alone suffices. However, there is a gray zone where the cause of the murmur remains uncertain. Two approaches are possible:

  1. Thorough Evaluation: Including a cardiac ultrasound.
  2. Reevaluation After Three Weeks:
    • If the murmur disappears, it was likely physiological.
    • If it persists, further investigation is recommended.

Conclusion

Detecting a heart murmur in a puppy is not necessarily a sign of a severe problem, but it deserves close attention. Regular monitoring is essential to ensure your companion grows up healthy with a strong heart.
